Applicants can obtain an application in advance at Bedford Community Resource Center, 403 Otey Street, Bedford, VA or from this web site.  Applications will also be available at the screening site.  Applicants must turn in the application in person at one of the screening dates listed above.  Applicants will need to bring proof of their MONTHLY income for ALL household members, and the following documents for each child in the home:  insurance card or birth certificate plus school documents if the child is in school.  School documents can include:  class schedule or room assignment, progress report, report card, letter from school addressed to the parents of the student, or a school registration form.
